The Primary FRCA is the first major professional exam for anaesthetists in training, and it is known for the depth of applied basic science it demands. Understanding the structure early makes preparation far less daunting.
What the Primary FRCA involves
The Primary FRCA has a written MCQ component, an OSCE, and a structured oral examination (SOE). The written paper is where most candidates start, and it tests pharmacology, physiology and biochemistry, and physics and clinical measurement, which is the science that underpins safe anaesthesia.
What the MCQ paper rewards
The written paper goes deeper than most postgraduate exams into mechanisms: receptor pharmacology, pharmacokinetics, respiratory and cardiovascular physiology, and the physics of measurement and equipment. Surface-level revision does not cut it; you need to understand the underlying principles well enough to apply them to unfamiliar stems.
An efficient preparation approach
- Anchor your revision in high-volume question practice mapped to the three science domains.
- Treat physics and clinical measurement as high-yield, since it is heavily tested and often under-revised.
- Use explanations to build mechanistic understanding, then use spaced repetition for the dense factual content (drug data, constants, equations).
- Practise applying principles to novel scenarios rather than memorising past questions.
